Most of those situations involve the physician failing to get all of the facts that he or she is going to need to accurately diagnose your condition. Did they listen to everything you told them? Did they take into account everything that's in your medical record? A lot of times what happens is the physician gets in a hurry, doesn't get all of the information and the end result is a wrong diagnosis.
